Evaluation of WHO-GSED for individual-level-assessment: Identifying and managing children under 3-years with or at risk of neurodevelopmental
impairment–Model for improved implementation of Rashtriya-Bal-Swasthya-Karyakram

Background:

Worldwide, 52.9-million children under the age of 5 experience developmental-impairments, India ranks-high with 12% preschool children developmentally impaired. Early-identification/intervention can improve the outcome. In recognition of this GOI has initiated Rashtriya- Bal-Swasthya-Karyakram (RBSK) with plans for early identification /management of developmental delays and disorders. Critical gap in implementation preventing successful implementation has been lack of screen tool and rehabilitation care packages implementable at community and primary care level. This gap is aimed to be plugged by recently validated Global-Scales-for-Early-Development(GSED), developed by WHO.

The scale is currently being validated for individual level assessment to identify NDIs. Hilton Foundation has funded Tanzania and Pakistan but India being non-LMIC, needs national funding. Validation in India would greatly benefit  materialization of objectives of RBSK. 

Innovative Development app based tool can be used by frontline workers (ASHAs or AWWs) easily enabling early screening at community level. mortality.

Study Setup:

Objective 1: Establish diagnostic accuracy of GSED tool in identifying and/or predicting NDI

Objective 2: Creating a rehabilitation package for NDI children that can be integrated into RBSK.

Objective 3: Contribute to the development of GSED-2.0, a reduced version with more sensitivity/specificity.

Objective 4: Estimate burden of NDI among children aged 3-36 months in Meerut, UP. Methods: A prospective community-based cohort, diagnostic-test evaluation study. Children evaluated by gold-standard-diagnosis of NDI and tested with GSED as investigative test.

Expected outcomes:

Sensitivity/Specificity and Diagnostic-accuracy for GSED package to identify children with/or-at-risk for NDI.

Development of careplan/training-package for rehabilitation for peripheral worker/care giver.
